UK AI adoption has gone mainstream while oversight has not kept pace. Drawing on 27 primary sources, this report quantifies the distance between deployment speed and the board accountability, policy, risk registers and assurance needed to deploy safely — across six priority sectors.

We read the most recent published annual report, governance statement or trustees' report of 150 large UK organisations across five sectors and scored what they say about AI against a ten-point rubric. Of the 147 we could score, 66.7% name AI, 7.5% record it as a standalone risk with a mitigation, and 1.4% name an owner. Every non-zero score carries a verbatim quote, published in full as a CSV.
